I'm playing as Charlemagne and even though the decision is displayed and nominally I fulfill all conditions (aka have at least 25 relations with the pope, have two years worth of income, no other Christian ruler is emperor of France, Britain, Germany or Spain, is an adult, is not incapable, is not a prisoner, prestige of at least 1000, real size of at least 220, emperor or king of Italy hold at least one kingdom or empire title in either France, Germany, Spain or Britain (I have several, I have West Frankia, East Frankia, Middle Frankia and Saxony) I can not create the empire. I'm running the Historical Immersion Project Mod but they haven't gotten back to me yet and maybe someone else who is only running the Vanilla version has the same issues. So I'm not 100% sure this is a problem of the base game itself.
